If you have a scheduled nutritional assessment, please use the link below to download and print your New Patient Questionnaire. Complete the form prior to your appointment and bring it with you on the day of your visit.
New Patient Questionnaire – Nutritional Assessment
Please have completed all your initial paperwork BEFORE your scheduled appointment. Also, please bring the following:
- Current prescriptions and/or over-the-counter medication, including dose and frequency
- Current nutritional supplements and daily dosages
- Recent test results, x-rays, or relevant records
Payment and Insurance Information
In order to keep our services as affordable as possible, our office operates on a fee-for-service basis. Please see our accepted insurances below. We are out of network with all other insurance companies. Payment is expected at the time of your visit, and a superbill receipt will be provided upon request for you to submit to your insurance provider.
